Anthem Urgent Care Copay



Anthem MediBlue Plus (HMO) H3447-021 is a 2021 Medicare Advantage Plan or Medicare Part-C plan by Anthem HealthKeepers available to residents in Indiana. This plan includes additional Medicare prescription drug (Part-D) coverage. The Anthem MediBlue Plus (HMO) has a monthly premium of $0 and has an in-network Maximum Out-of-Pocket limit of $4,400 (MOOP). Then, you can follow the steps until you reach the insurance information page. The price you pay will either be your insurance copay or the low cash price of $79. You have the option of cancelling the visit, if for some reason your insurance does not cover it.

When The Ashe Center is closed, you still have access to care -- both online, and in the local community using your UCSHIP benefits. Our 24/7 Nurseline is available After Hours, LiveHealth Online offers telemedicine visits online, and The Ashe Center keeps a list of local urgent care centers updated for your convenience.

We realize that getting sick is not always predictable. In the event that you need to seek medical care when The Ashe Center is closed, remember that all currently registered UCLA students have access to various alternatives for urgent access to medical care, including: 24/7 Nurseline, telemedicine visits through LiveHealth Online, local urgent care centers, and in the case of an actual emergency, emergency room access.

Weekend Telehealth Visits (NEW!)

Ashe Center primary care providers are now available for weekend telehealth video visits during the COVID-19 emergency. While social distancing and other measures to prevent infection are of the utmost importance, it's equally important to stay healthy and keep any pre-existing medical conditions under control. Instead of coming to see providers in person for an exam, consultation, or follow-up appointment, you can connect with your PCP using a HIPAA-compliant version of Zoom on your mobile device, tablet or computer. The information is securely transmitted and is treated the same way it would be if you were meeting with your provider in person. To schedule, log into the Patient Portal and select Weekend Telehealth Visit. Telehealth appointments available 9am-4pm on Saturday and Sunday.

Accessing Urgent Care Centers

In-network (Anthem Blue Cross) urgent care visits have a $25 co-payment.

Note: If you go to a non-participating Anthem Blue Cross urgent care center, deductible will be applied and the claim will be covered at 60 percent of the maximum allowed amount. You may be held liable for any amount over the maximum allowed amount.

Uber Ride Vouchers for In-person Weekend Care

The Ashe Center is pleased to work with Uber to assist students with urgent needs when our center is closed. Click an entry below for the requested date and Uber will email you a unique ride voucher to travel from The Ashe Center to UCLA Health Century City. Vouchers are available for each weekend clinic date of the quarter and will be audited to ensure proper use.

In Case of Emergency

Emergency room visits at participating Anthem in-network facilities have a co-payment of $125. If you are admitted, this co-payment will be waived and you will be charged the hospitalization co-insurance rate. Please visit the Plan Highlights for additional breakdown of these fees.
Additionally, the UCLA Emergency Room can be reached directly at (310) 825-2111. Remember that it is important to follow up with your Ashe Center Primary Care Provider (PCP) at least 24-48 hours after being seen in an urgent care facility to ensure continuity of care.

For mental health-related concerns when the Counseling & Psychological Services (CAPS) is closed, you may speak with a CAPS Crisis Clinician by calling (310) 825-0768.

UC Care is a PPO plan created just for UC. You can get care from most UC physicians and medical centers as well as the Anthem Preferred network of providers — the choice is yours. You also have coverage for non-network providers.

Plan changes for 2021

Effective Jan. 1, 2021, there are changes to member cost-sharing amounts:

  • The calendar year deductible remains $0 when you see UC Select providers. For other providers, deductibles are increasing to $500 (preferred)/$750 (non-preferred) for individuals and $1,000 (preferred)/$1,750 (non-preferred) for families. The deductible is the amount you pay before the plan begins to share in the cost for covered services.
  • The copayment for urgent care for Anthem preferred providers has decreased from $30 to $20. The copayment for emergency care has increased to $300 if the patient is not admitted to the hospital.
  • Coinsurance for Anthem preferred providers has increased from 20% to 30%.
  • The out-of-pocket maximum has increased to $6,100 (UC Select)/ $7,600 (preferred)/$9,600 (non-preferred) for individuals and $9,700 (UC Select)/ $14,200 (preferred)/$20,200 (non-preferred) for family coverage.

New ID Cards

All UC Care members will receive a new ID card from Anthem by the end of December 2020 to use beginning Jan. 1, 2021, for medical, prescription drug and behavioral health services.

How the plan works

  • You may choose any doctor or care facility, worldwide.
  • You have two options for in-network care:
    • You pay set copayments for covered services and there’s no deductible when you use providers in the UC Select network (available only in California).
    • You also can choose a medical provider in the Anthem Preferred network and pay 20% of the cost of service after the deductible has been met. The Anthem Preferred network has a $250 deductible for individual coverage and $750 deductible for a family of three or more.
  • Or, you can choose a non-preferred or out-of-network provider and pay 50% of the cost. There is a $500 deductible for individual coverage and $1,500 deductible for a family of three or more.
  • Behavioral health benefits are provided through Anthem Blue Cross. You can see any behavioral health provider you choose, but you’ll pay less and receive higher benefit coverage when you see an in-network provider. Some services may require prior authorization.
  • Annual out-of-pocket maximums limit what you pay. If you reach the annual maximum, the plan pays 100% of your covered medical costs for the rest of the year. There is one combined out-of-pocket maximum for medical, behavioral health and pharmacy expenses.
    • UC Select: $5,100/individual; $8,700/family
    • Anthem Preferred: $6,600/individual; $13,200 family
    • Out-of-network: $8,600/individual; $19,200 family
  • Anthem Blue Cross will administer claims.
